This pre-registration/orientation
event is held for students who will be entering kindergarten
in the fall or for students who will be attending first grade
and who have attended an approved kindergarten.
Parents are requested to accompany
their children to the school where they will be attending next
fall. The round-up is designed to acquaint students and parents
with schools and to provide information to parents concerning
entrance requirements.
If you will be unable to attend,
you are requested to phone the principal of the school and give
pre-registration information concerning your child. It is important
that the school system obtain a relatively accurate census of
students entering school so that plans can be made to serve the
students' needs.
The following will be required for
all children entering school for the first time:
To obtain a copy of the birth certificate,
you may contact the Health Department of the state your child
was born or you may contact the Vital Records Department of the
state where your child was born. Include: father's name, mother's
maiden name, child's birth given name, county of birth, and date
of birth. In Tennessee, this must be submitted on the official
request form that can be obtained from your local health department.
Payment must be sent with your request. The charge for the short
form is $5.00, long form is $10.00, each additional copy is a
charge of $2.00. Phone requests will be charged to a credit card
(VISA, Discover, American Express, or Master Card) for an additional
$10.00. The credit card number is 615-741-0778. The direct number

Social Security numbers are requested
on all students entering school for the first time.

Tennessee school entry medical
examination. The school system will usually accept physicals
done during March through August of the current year. The school
system encourages parents to get these physicals completed
early in order to have them ready when school starts.

Evidence of state-required immunization
which shall be the permanent (green) or the temporary (white)
certificate. A child will be issued the permanent certificate
if he/she has had the following:
DTP-DTaP-DT-DTP/Hib-Td
4 or more doses, one of which was given
on or after the fourth birthday. If age seven or older three
doses are required.
Polio (OPV/IPV)
4 doses of OPV or IPV. If the 3rd dose
was given on or after the fourth birthday, the 4th dose is not
required. However, if a combination of IPV/OPV (2IPV/2OPV) is used,
all 4 four doses are needed regardless of age.
MMR (Mumps, Measles, Rubella)
2 doses, given on or after the first birthdayrequirted,
for entrace into grades K, 4, 8, and 12. The dose should be administered
at least 30 days following the first dose.
Hepatits B
3 doses, for entrance into kindergarten
(effective July 1, 1999).
A child will be issued a temporary
certificate, if at least one dose of each vaccine has been administered.
The temporary certificate will not be valid seven days after
the next dose is due.
Any child who is five (5) years of
age on or before September 30 of the current year, may enter
kindergarten. Any child who is six (6) years of age on or before
September 30 of the current year, may enter the first grade if
he/she has attended an approved kindergarten.
